Founded and incorporated in 1988, Khaleidoscope Health Care initially provided only comprehensive case management for its clients, but has since been licensed as an ambulatory primary care provider (primary and internal medicine, ob/gyn and pediatric services) and substance abuse treatment provider specializing in addictive services. Khaleidoscope Health Care, Inc. is the only ambulatory health care agency in the State of New Jersey to hold this dual licensing.
Khaleidoscope Health Care provides the medical attention that its patients sorely need and the treatment many individuals require for their dependencies, in a clean and cheerful atmosphere that is respectful of them as peoplenot just as patients. In doing so, Khaleidoscope works to make the lives of its patients healthier and richer so that they may be participating, contributing members of their families and their communities.
Khaleidoscope operates from two sites in the middle of a predominately minority community. One is a 1,000 square-foot facility at 127 Lafayette Street (Parkside Medical Center) where affordable primary health care is offered. KHC also operates a full-service ambulatory health center at 75 Harrison Avenue that specializes in addictive care treatment.
