Notice of Confidentiality of Alcohol and Drug Patients:
The confidentiality of alcohol and drug abuse records maintained by Khaleidoscope/Parkside is protected by Federal laws and regulations.Counselors and staff members may not tell anyone outside of the program that a patient attends the program, or disclose any information identifying a patient as an alcohol or dug abuser unless: (1) the patient consents in writing; (2) the disclosure is allowed by a court order; or (3) that disclosure is made to medical personnel in a medical emergency or to qualified personnel for research, audit or program evaluation.
Violation of these Federal laws and regulations is a crime. Any person who violates any provision of this regulations is subject to a fine of $500.00 for the first offense and up to $5,000.00 for each subsequent offense.

Federal laws and regulations do not protect information about suspected child abuse or neglect from being reported to appropriate State or local authorities.
