An adult examination (15 years or older) usually includes:
1. Physical Examination;
2. Tuberculin (TB) skin test;
3. Blood test for HIV (the virus that causes AIDS);
4. Blood test for syphilis (RPR);
5. Blood test to determine immunization levelsMeasles, Mumps, Rubella, Tetanus/Diphtheria, Varicella, and Influenza vaccines.
A pediatric examination (children 14 years of age or younger) does not usually require blood testing and usually includes:
1. Physical examination;
2. TB skin test (2 years and older);
3. Review and update of the pediatric immunizationsRotavirus, Measles, Mumps, Rubella, Tetanus, Diphtheria, Pertussis, Polio, Haemophilus Influenza Type b, Hepatitis B, Hepatitis A, and Pneumococcal vaccines.
Please bring the following items with you:
1. Picture identification such as a passport or other Federally approved documentation (State identification such as a driver's license and school ID is not acceptable);
2. Your medical records for any chronic or serious medical problem;
3. Your immunization records, if available;
4. Alien Registration number, if you have one;
5. I-693 medical form, if you have one (or, we can provide it to you);
6. Supplement to Form I-693, if you have one (or, we can provide it to you).
If your medical records are not in English, please have them translated before your appointment.
Our staff speaks English, Spanish and Mandarin Chinese.
Costs:
$355 for adult’s physical examination plus additional charge for any immunizations that must be provided; $200 for child’s physical examination plus additional charge for any immunizations that must be provided.
We accept VISA, MasterCard, and Debit Cards as well as cash, and money orders. All expenses must be paid at the time of the examination.
