Khaleidoscope Health Care, Inc. is a dually licensed and accredited ambulatory health care provider that also offers outpatient and intensive outpatient (IOP) substance abuse treatment.

This means patients can access primary medical care and behavioral health services in the same organization, with a team working together to support their overall care.

As a 501(c)(3) nonprofit organization, Khaleidoscope's primary mission is to provide access to quality, affordable, community-based health care through education and preventive medical care. We are dedicated to serving the unmet health care needs of underserved residents throughout Hudson County.

Primary & Preventative Care

Khaleidoscope is a dually licensed ambulatory health care provider, which means we're not only an addiction treatment program — we're also a medical clinic. Our providers deliver primary care to adults, seniors, and young adults throughout Jersey City and Hudson County, whether or not you're enrolled in any of our behavioral health programs.

Our medical team provides a range of services to help patients manage their health, stay on top of routine care, and address concerns early, including:

  • Internal medicine

  • Physicals and routine examinations

  • Laboratory work

  • Pregnancy testing

  • Nutritional counseling

  • Family counseling

  • Workers' compensation examinations


Our prevention-focused approach is deliberate. Routine care is often the first thing people put off when money is tight, work is unstable, or life becomes difficult. But regular checkups and preventive care can help identify health concerns early and give you the support you need to stay well.

We also provide nutritional counseling and education focused on healthy living. Our goal is to make preventive healthcare practical and accessible while helping patients develop healthier habits that support their long-term well-being.

Specialized Care for Our Community

We are committed to providing respectful, accessible care for people with different health needs, backgrounds, and circumstances. Our services include dedicated support for:
 

  • People living with HIV or AIDS

  • Pregnant and postpartum women

  • Women's health needs

  • Specialized care for LGBTQ+ patients

  • Seniors and older adults

  • Clients referred through the court or judicial system
     

Spanish-language services are available, along with ASL and other assistance for individuals who are hearing impaired.
 

Because Khaleidoscope provides both medical and behavioral health services, patients who receive multiple types of care can have their needs addressed within the same organization. This can make it easier for providers and counselors to understand the bigger picture and coordinate care.
 

With two Jersey City locations, most insurance accepted, and sliding-scale options available, Khaleidoscope works to make primary and preventive care accessible to the Hudson County community.

Substance Use Treatment

Khaleidoscope Health Care is a SAMHSA-certified Opioid Treatment Program (OTP) providing treatment and recovery support for adults with substance use disorders. Our programs are designed to meet patients where they are and provide different levels of care based on their individual needs.
 

Treatment Programs & Services
 

Our substance use treatment services include:
 

  • Medication-Assisted Treatment (MAT) — Medication-based treatment for opioid use disorder, including methadone, buprenorphine (Suboxone), and extended-release injectable naltrexone (Vivitrol).

  • Intensive Outpatient Program (IOP) — Structured treatment that includes individual counseling, group therapy, treatment planning, and intensive services while allowing patients to continue living at home.

  • Regular Outpatient Treatment — Flexible counseling and recovery support designed to work around work, school, family, and other everyday responsibilities.

Mental Health & Co-Occurring Disorders

Khaleidoscope provides integrated care for adults experiencing both mental health and substance use concerns. Our approach recognizes that these conditions can affect one another and that addressing both together can provide more coordinated and effective support.

Integrated Care
 

Mental health and substance use concerns are addressed together rather than treated as completely separate issues. By providing behavioral health, substance use treatment, and medication management within the same organization, our providers can work together to better understand each patient's needs.
 

Our goal is to help patients develop healthier coping strategies, manage mental health symptoms, address substance use, and build a stronger foundation for long-term recovery.
